Need to complete the IL dental examination requirement for your children? No problem! Complete the parent/guardian portion of the form to the left and give us a call to schedule an exam. We are happy to perform a thorough examination and complete the remainder of the form for you.

We proudly accept most insurance plans; however, we are not currently in-network with any providers. This decision allows us to maintain the highest quality standards in dental care. By remaining out-of-network, we can continue to utilize the best materials, state-of-the-art equipment, and top-tier labs without being constrained by the lower reimbursement rates of in-network insurance plans. As a courtesy, we are happy to submit insurance claims on your behalf to help you maximize your benefits.
